Excellent mystery with a twist

An extremely good mystery with a twist. The protagonist is an old man in a nursing home -- realistic picture of aging process- shows body going, mind ok, and then treatment (lots of drugs). A good mystery -- and shows the reality of old age in North America -- overmedicated, trivilized, etc.
