Necessary Resource for Personal Health Management

I've purchased two of these Health Organizers in the last 10 years, and they have been invaluable in helping me manage my personal health care. My doctors often state that "this [organizer] helps [them] a lot", and use the notes to structure appropriate treatment plans. This organizer is an ESSENTIAL resource for people with Chronic conditions, for PARENTS who need to keep track of their children's health, CAREGIVERS taking care of family members, etc. It is clearly laid out. The language is plain and straightforward. There are no complex forms to try to understand, and the charts are extremely simple yet enable you to include all important information. It removes the stress of trying to remember all of the details yourself or to guess what you need to know to manage your care. It holds the full range of information required on any medical form: Personal information, Physician's name, specialty, contact, etc. (for several physicians, not just one or two); insurance they accept, co-pay information, etc., Charts for Medical History, Allergies, Diabetic monitoring, Weight charts, Billing / tracking, you name it. An extensive section for Medications Taken: Dosage, Date prescribed, prescribing physician, Side-Effects, Results, etc., Surgeries had, results, etc. There is a section (extensive) for Weekly monitoring, headed by your weekly goals, followed by dated lines where you can record your health goals/symptoms, or whatever related details you want. I cannot describe enough details here, but again, the book is ESSENTIAL for health care management. Note: These details are recommended for Emergency Medical Care should you go to an ER: you or someone else can get an instant overview of your medical situation, and it can help the administrative process. Books are more convenient than electronic files, for, should you be unable to speak, a caregiver won't have to try to find your password to access a computer file, etc., they can simply refer to the book and see how to help you right away. Health Insurance Companies can use the information to track billing matters, pre-authorization histories, etc. Social Security can use the information to help determine benefits for you. It is a well-written book and I recommend it highly.
