This book is a homely extension of an article of the same name by Everett C. Hughes. In fact, Hughes' article is appended to the book chapters. It is intended to show that mistakes at work are common and to be expected and do not have to be explained as deviations. It also takes up issues of the way mistakes are typically handled by workers, under what circumstances they are most likely to occur, when the workers know mistakes are being made, the control of the definition of mistakes, and who pays for mistakes.
